Job Description

Lighthouse Trust is a registered Public Trust established to manage Centres of Excellence (CoEs) and provide leverage for leadership and delivery of integrated HIV prevention, treatment, care, and support services in Malawi. Lighthouse has been involved in the development of locally applicable HIV care and treatment models. It is strategically positioned in Malawi’s national HIV response due to its close collaboration with the Ministry of Health (MoH). The Lighthouse in close coordination with MoH provides integrated services at Kamuzu Central Hospital (KCH), Bwaila Hospital and some health facilities in urban and rural areas in Lilongwe, Queen Elizabeth Central Hospital (QECH) in Blantyre, Zomba Central Hospital (ZCH), Mzuzu Central Hospital (MCH) and District Support Program (DSP) for Northern Region districts. Its strategic approach focuses on client-centred care based on four goals which are:

Goal 1: Service delivery: to enhance high-quality, integrated HIV prevention, treatment, care and support services for adults and children


Goal 2: Model interventions and operations research: to develop, pilot, evaluate, and disseminate innovative programs and policies across the continuum of prevention, treatment, care and support for HIV infection and its co-morbidities, both infectious and non-infectious.

Goal 3: Quality management: to ensure that all patients at Lighthouse clinics and all supported sites are receiving quality health care and services as recommended by the MoH and other health development partners by utilizing quality improvement

(QI) methodologies in a multidisciplinary team approach.

Goal 4: Capacity strengthening: to build and strengthen healthcare capacity for high quality HIV prevention, treatment, care and support services.

Lighthouse Trust, with support from the United States of America President’s Plan for AIDS Relief (PEPFAR) through a Cooperative Agreement with Centres for Disease Control and Prevention (CDC), has two grants to operate Centres of Excellence for Comprehensive Integrated HIV Care and Treatment Services in Lilongwe (Grant #NU2GGH002131), and, also to achieve HIV Epidemic Control through Scaling up of Quality Testing, Care and Treatment in sites outside Lilongwe

(Grant # NU2GGH002003). In order to achieve the above-mentioned goals, Lighthouse wishes to recruit a qualified and experienced person to fill the following position:

Purpose of the Position


  • The primary purpose of the position is to strengthen the Lighthouse’s role as a Centre of Excellence (CoE) for integrated HIV Prevention, Treatment, Care and Support of the national HIV programs in Malawi aligned with the 95-95-95 framework and the National Strategic Plan (NSP) for HIV. The Medical Director will lead the development of strategies and guidelines that are evidence-based and
    maximize impact on quality HIV care cascade for adults, children, and Option B+ women, from enrolment to ART initiation, adherence, and retention in care and integration models.
  •  The incumbent will provide leadership on the coordination and oversight of all Lighthouse Medical Services and will ensure continued development, implementation, and evaluation of the service delivery technical approach models and ensure proper linkage with the operations research and
    capacity building technical approaches. The Medical Director will ensure continued delivery of quality health services to all clients accessing services from Lighthouse Clinics and its supported facilities. In this role, the Medical Director will serve as the lead providing strategic medical guidance to Lighthouse Clinics, HIV Testing and Counselling Services both at the central and district level, and Community Health Care Services.

Key Duties and Responsibilities

(a) Medical Services Management Functions

  • Improve the quality of care for prevention and management of HIV associated conditions.
  • Strengthen the basic packages of HIV care and support, diagnostic support and interventions that are appropriate to HIV care clinics.
  • Address health systems bottlenecks that impact on the quality of clinical care, retention, and attainment of viral suppression.
  • Strengthen linkages between HIV care, support and other HIV prevention services.
  • Integrate other medical services into ART’s overall comprehensive services.
  • Perform clinical supervision of medical staff.
  • Lead the planning for the new clinical programs.
  • Provide counsel in personnel matters relating to the medical staff.
  • Constantly review and revise the Lighthouse Clinical Manual of Standard Operation Procedures that include ART policies, protocols and practice guidelines and referral mechanisms.
  • Conduct operations research and evaluations utilizing sound research methods in collaboration with the Clinical Advisor and the Monitoring, Evaluation and Research Director.
  • Establish a case management process that fosters cost-effective, high quality patient care.
  • Attend meetings on Technical and Policy working groups.
  • Organize, coordinate, and monitor the activities of the medical staff and ensure that the quality and
    appropriateness of services meets required standards.
  • Make certain that medical records systems meet the needs of clients and are operating as required.
  • Contribute to the writing of technical reports for submission to the Executive Director.

(b) Administrative Functions

  • Maintain an effective working relationship with other departments within Lighthouse.
  • Participate in the development and writing of funding proposals.
  • Attend Board of Trustees, Executive and Management meetings.
  • Take part in the recruitment processes for medical personnel.
  • Participate in Internal Procurement Committee meetings especially on procurement of medical related equipment and supplies.

(c) Performance Improvement Functions

  • Oversee and maintain performance improvement processes within the clinic, HIV Testing Services and Community Health Services.
  • Review patient feedback from different methods and sources.
  • Identify and correct deficiencies in ART care policies, guidelines and protocols.
  • Take part in the monitoring of care within the facility through quality assurance program that encourages self-evaluation and plans for change and meets regulations.
  • Monitor Health worker’s performance and adherence to quality assurance standards.

(d) General

  • Demonstrate an understanding of Lighthouse’s mission in performing all aspects of the position.
  • Exhibit a caring and helpful attitude when interacting with clients, fellow employees and other collaborators, and strive to build cooperative partnerships with internal and external customers.
  • Assist in promoting an environment in which the medical providers retain independent judgement and responsibility in the practice of medicine, subject to peer review by and recommendations from the Clinical Advisor.
  • Provide an environment in which the medical providers follow and abide by the ethics of the medical profession, all applicable MoH laws and ordinances, and all other policies adopted by MoH.
  • Handle confidential information as required.
  • Contribute in other program activities as appropriate.

(e) Supervision and/or Leadership Functions

  • Supervise the Chief Programs Managers.
  • Oversee all the Centres of Excellence in Lighthouse.

(f) Decision Making Authority

  • Should be capable of making decisions which will be efficient and effective to the Directorate.

(g) Knowledge, Skills and Abilities

  • Excellent communication and writing skills in English.
  • Excellent collaboration and partnering skills in a multidisciplinary environment.
  • Ability to manage multiple priorities and projects while working as part of a team.
  • Ability to work under pressure.

Academic Qualifications and Experience

  • MBBS/MD from an accredited Medical School with relevant Post Graduate Training (e.g. Internal medicine and/or Master’s in Public Health or Health Systems Management).
  • Certified by the Medical Council of Malawi.
  • At least 8 years of relevant work experience in HIV Program Management.
  • Strong experience in providing mentorship and technical assistance.
  • Self-motivated and demonstrated success in building effective programs.
  • Experience in Operations Research is desirable.
